Kelly Park Limited is a passionate, family-run home care provider based in Consett, proudly supporting people across County Durham to live safely and independently in their own homes. As one of the preferred provider for Durham County Council, we specialise in delivering person-centred, compassionate, and high-quality domiciliary care.
We are now seeking a dynamic, organised, and committed Home Care Coordinator to join our growing team. This is a vital role where you'll directly impact the quality of life for both service users and staff, ensuring that care is delivered reliably, professionally, and with genuine empathy. You’ll be responsible for overseeing care delivery within a designated area, managing a high volume of scheduling hours, and acting as a supportive link between clients, caregivers, families, healthcare professionals, and local authorities.
Responsibilities
As a Home Care Coordinator, you will:
- Plan and manage over 1,000+ hours of domiciliary care schedules each week across a designated territory.
- Use our digital resource planning software to schedule care at least two weeks in advance based on staff availability, client needs, and contractual obligations.
- React swiftly to changes in packages, care plans, staff availability, and emergency cover – always ensuring continuity and quality of care.
- Build and maintain strong, trusting relationships with service users and their families, championing a person-centred approach at all times.
- Liaise with a variety of stakeholders including local authorities, contracts teams, hospitals, and family members to maintain safe and efficient service delivery.
- Oversee the day-to-day running of care operations in your assigned area, including the governance and allocation of villages and rural settings.
- Provide on-call telephone support during out-of-hours on a rota basis (early morning and evening coverage paid in addition to your salary).
- Work closely with Care Coordinators, Field Supervisors, and Registered Managers to maintain service standards and resolve operational challenges.

Qualifications
- Full UK driving licence and access to a vehicle (essential).
- Excellent I.T. skills, particularly with scheduling software and Microsoft Office.
- Strong administrative and customer service abilities.
- Previous care sector experience (essential), ideally in a coordination or supervisory role.
- NVQ Level 3 in Health and Social Care or equivalent (desirable).
- Enhanced DBS check (required prior to appointment).
- Two satisfactory references.

Experience
- Experience in a care coordination, rota planning, or team-leading role within a domiciliary care setting is essential.
- Proven track record of managing scheduling logistics and maintaining service compliance.
- Experience of dealing with external stakeholders including healthcare professionals, local authorities, and family carers.
Shift Patterns & Area Responsibility
- Monday to Friday: 08:30 AM – 5:00 PM
- On-call support: Rota-based cover (07:00–08:30 AM and 5:00–11:00 PM) — paid as an additional enhancement
- You will oversee the governance and coordination of services within assigned villages and surrounding areas.
