Job Title: Carer Support Adviser

Salary range: £19,054 per annum (£23,500 FTE)

Contract: 12 months Fixed-term Maternity Cover

Hours: 30 hours per week

Location: Flexible and hybrid working in Medway with community working

About this role

As a Carer Support Adviser your responsibilities would include: 

• Managing conversations with carers to identify needs with assessments and support plans

• Provide advice and wellbeing support

• Run support groups and activities

• Work with community groups and partnership working to identify Carers in Medway

About you 

To be successful in this role you will need

• Experience of working with adults either in a 1-1 or group setting

• Working knowledge of health and social care particularly in relation to adults

• Excellent communication and interpersonal skills

• A high standard of written skills is needed, in order to help the Carer identify their support needs and build resilience
